APPLICATION FOR A STUDY PERMIT- CHECKLIST
· Complete and place this checklist on top of your application.
· Consult or website for forms and instructions, including a list of countries whose national require a temporary resident visa at www.flytdtravel.com
· Any document not i9n English or French must be accompanied by a translation.
· Failure to submit all required documentation may result in the refusal of your application or delay in processing
· False statement or submissions of fraudulent document will result in refusal and may lead to legal action against you.
· All document submitted in support of your application should be original.
· After submission of all required document you may be required to attend interview.
· A medical exam will be required for study program, exceeding 6 months.
· Do not undergo a medical exam until advised by the office. Medical result may take up to 4 weeks.
· Any preparation you make, such as paying administration or tuition fees, are done entirely at your own risk.
· Do not finalize travel plans unless you have received your visa from Canadian Deputy High Commission in Lagos.
· A parent or legal guardian will sign for a minor child.

Each applicant and each accompanying family member must complete, date and sign a separate application form. NOTE: it is recommended that additional family members not make application until such time as the principal applicant has been processed.
List your spouse/common la-law partner an ALL your children under 18 years of age (or still dependent) on the IMM 1294 form, even if they do not need a visa or not accompanying you. If you have more than three family members, fill additional copies of the IMM 1294 form.
Answer every question. If not applicable, write N/A. make sure you include your mailing address, reliable phone contact, and email address (if you agree to e-mail communications).
It is advisable to fill out the new forms electronically and submit a print-out of your computer generated forms, including the 2D barcode page.

|Processing fee – ensure that you have paid the correct non-refundable fee in naira ( bank draft only) made payable in Lagos to the “Canadian Deputy High Commission”. The passport of the principal applicant must be printed on the back of each draft.||
|Original passport for yourself and each family member requesting a visa, including children. Each passport should:
X be accompanied with a copy of the passport bill-date page ( i.e. the page with the name, photo, place of birth, date of issue, etc.)
X show that you are a citizen or lawfully admitted to your country of residence (e.g. Study permit, work permit)
X be valid for the duration of your study period as a study permit cannot be Issued beyond the validity of your passport
X have a minimum of two blank visa pages.

Original and 1 Photocopy of Letter of Acceptance from your educational Institute in Canada starting:
x unconditional admission, or if conditional you must show you can meet the conditions
x course title and level of study,
x duration of study, AND
x start date (multiple start dates, if applicable) and latest possible dates of admission.
Academic Documents – originals and 1 photocopy of:
x present academic standings (up to date transcripts);
x WAEC/SSCE/NECO results;
x post secondary transcripts;
x all certificates and/or diplomas. You must provide evidence of all current and past education,
including the street names address of ALL previous educational institutions that you attended.
x Include NYSC certificate if applicable.
Proof of sufficient funds to cover your tuition and living expenses for at least one year in Canada.
In addition to the cost of tuition, you should have at least CAD $10,000 available to cover your
living expenses; an additional minimum requirement of CAD $4,000 must be added if a spouse or
partner is accompanying the student, as well as CAD $3,000 per accompanying child or other accompanying dependent.
x Original plus one copy of certified bank statement(s) or financial investment with past six months history including your submission month or ending one month prior,
x You must also show a reliable source of funds for the duration of your academic program.
x You may also submit financial documents related to a valid pension plan or proof of income from
rental properties or stocks and bonds.
x Failure to provide evidence may result in refusal for “insufficient funds.
Scholarships or any financial assistance from the school or award from a government or other
funding agency must be presented on official letter stating the full amount of the award, its
duration and any governing conditions. There must also be evidence a Scholarship program exists
Payment of Tuition fees is not required at time of application submissions. After positive
Processing of the student application, proof of tuition payment may be required prior to the issue
of a Visa.
Official Fee Schedule which includes – tuition, accommodations and meal plans, miscellaneous expenses (medical insurance, books, memberships).
Original police clearance certificates obtained from C.I.D Headquarters for the applicant and any
accompanying family member above 18 years for Nigeria. Certificates are also required for each of
the countries in which they stated more than 6 months since they turned 18.
Self-addressed pre-paid return courier envelope to return your passport and documents whether
or not you are submitting your application by courier service or drop box.
If applicable, you must also submit:
Certificate d’ Acceptation du Quebec (CAQ) if you will be studying in Quebec.
- From the individual (s) providing you financial assistance for your program. The letter must
Indicate their employment; their occupation; their relationship to you, their number
Dependents and full contact details. For extended family member sponsorship please include
proof of relationship (Such as family tree, photos, proof of prior financial support of your
education to date if applicable. Etc.
- From the employer of the individual(s) providing you financial assistance for your program. employer contact details and full street address of business’ location.
Proof of business if you are self employed. Provide business registration, tax clearance certificates, trade certificate, license and financial documents. (Certified business bank statement with 6 months
History up to application submission date).
Proof of employment – letter from your current or past employer and payslips (last 3 months).
Employment letter dated within 2 months of the application indicating occupation, job description,
Position, salary, allowances, dates of employments and authorized leave dates.
Marriage Certificate and Birth Certificates for each family member whether or not they are requesting a visa.
Custodianship documents are required for students who are under the age of majority in Canada.
The age of majority is 18 in Alberta, Manitoba, Ontario, prince Edward Island, Quebec and
Saskatchewan. The age of majority is 19 in British Columbia, New Brunswick, Newfoundland, Nova
Scotia, Northwest Territories, Nunavut and Yukon.
Minor applicants under 18 years of age, travelling with only one parent or without parents, need
Custody documents from the other parent or both parents, as applicable.
Custodian Declaration – All minor applicants must supply a notarized declaration, signed by the
Parents or legal guardian in the country of origin and signed by the custodian in Canada, stating
Arrangements have been made for the custodian to act in place of the parent (s).
Children must also have a school leave letter, if applicable.
If you would like Citizenship and immigration Canada (CIC) to release information from your case file
to someone other than yourself, you must authorize us by completing Authority to release personal information to a designated individual (IMM 5475).
If someone is assisting you in completing your application or you would like someone to do business
with our office on your behalf, complete Use of a Representative form (IMM 5476).
